Growing up with a dentist stepdad, Cheryl Burke of Dancing with the Stars heard a lot over the years about the importance of good oral hygiene — in particular, the benefits of using dental floss.

“My dad would say, ‘make sure you floss,’ but I never really listened to him. I was very, very stubborn,” Cheryl told Dear Doctor magazine recently in an exclusive interview. Cheryl admits this stubbornness took its toll, in the form of tooth decay. “I definitely had my share of cavities,” Cheryl recalled.

Cavities can form when food particles, particularly sugar and carbohydrates, are not effectively cleaned from the spaces between teeth. These particles are then broken down by bacteria naturally present in the mouth, resulting in the production of acids that attack the tooth enamel.

When she reached her twenties, Cheryl decided she really needed to step up her oral hygiene and cultivate an asset so important to a professional dancer: a beautiful smile. And once she did, cavities became a distant memory.

“I think when you do floss frequently, it helps to reduce the chances of getting cavities,” Cheryl said. “It took me a while to figure it out.” Now Cheryl flosses after every meal. “I carry floss with me wherever I go. I have no shame busting out my floss in the middle of a restaurant!” She declared.

Dental decay is actually a worldwide epidemic, especially among kids. Untreated, it can lead to pain, tooth loss, and, because it is an infectious disease, it may even have more serious systemic (whole body) health consequences. The good thing is that it is entirely preventable through good oral hygiene at home and regular professional cleanings here at the office.

If it has been a while since you or your children have seen us for a cleaning and check-up, or you just want to learn more about preventing tooth decay, please call us today at 518-237-0019 to make an appointment for a consultation. If you would like to read Dear Doctor's entire interview with Cheryl Burke, please see “Cheryl Burke.” Dear Doctor also has more on “Tooth Decay: The World's Oldest & Most Widespread Disease.”

For a bride, there is so much to remember when planning the wedding — invitations, a dress, flowers, the site, the caterers, the cake, the photographer — the list goes on and on. In the midst of all these things, don't let a very important piece of the plan be forgotten — your smile!

Your wedding will be celebrated every year on your anniversary and commemorated in beautiful photos at any time. It is a special occasion you, your family, and your friends will always remember. But how will you and your family look in those photos? You have an opportunity to get makeovers that will make your smiles shine radiantly, and not only in your album. Your makeover will improve your appearance, your self-confidence, and your oral health far into the future.

Start with a visit to your dentist to assess your current situation and discuss your goals and dreams. Each person's situation will be different. At a minimum you may need a professional cleaning to make sure that your teeth and gums are healthy. Get started on treating any cavities or gingivitis (gum inflammation). If you haven't already, now is a great time to establish healthy dental hygiene habits including proper daily brushing and flossing.

You may feel that your teeth are discolored and need to be whiter and brighter. Your upper and lower jaws may not meet properly for the most effective biting and chewing. Your teeth may be crowded or misshapen. All these things and more can be modified and repaired with today's dentistry.

Teeth can be whitened by bleaching in the dentist's office or with products that can be used at home under a dentist's supervision.

Orthodontia may be needed to repair your bite. Today orthodontia may be done either with traditional braces, or with clear aligners. Remember that this process works slowly, so start well in advance of your wedding day.

After your teeth are properly aligned, your dental team can work on subtle contouring, overall shape and color changes for individual teeth, using techniques such as porcelain laminate veneers. Temporary veneers can be made so that you can try out your new look before the final installation.

Wedding planning works best when started early. To make sure you, your new spouse, and your family look and feel their best, remember to include smile makeovers in your plan.

Call us today at 518-237-0019 to make an appointment to discuss your questions about wedding smile makeovers. You can also learn more by reading the Dear Doctor magazine article “Wedding Day Smiles.”

If you are dissatisfied with the way your smile looks, and your dentist is unhappy with the way your teeth fit together — but you don't like the idea of wearing braces — clear aligners may be your best solution. How much do you know about this teeth-straightening alternative? Below are some FAQs on the subject.

What do we mean by clear aligners? Clear aligners are a system for straightening teeth that uses clear plastic removable “trays” that fit over your teeth. As the teeth move to fit the trays, new trays are substituted that are designed to continue to move your teeth into the desired position. This system is an alternative to the traditional system of brackets and wires known as braces.

How can teeth be moved to new positions? The connection that holds a tooth in place in your jaws — the periodontal ligament — is not immobile. It constantly changes its position based on the normal forces of your bite. As the ligament is pushed on one side and pulled on the other, the living cells of your mouth respond by depositing bone and cementum (the protective covering of the tooth's root) on one side and dissolving it on the other. Normally this happens in a balance, maintaining your teeth in their position. We can manage these slight changes by applying constant light forces to move teeth in a predictable way.

How long does it take to move teeth to their optimum position using clear aligners? As with braces, the process is gradual. Total treatment time can range from six months to two years.

Do the trays have to be worn all the time? As you move through the sequence of trays, each is worn for 20 hours per day for two weeks. They may occasionally be removed for important social occasions.

How does an orthodontist design the sequential trays that are used? The trays are designed using a computer, based on an assessment and images of your mouth, teeth and jaws.

What kinds of problems can clear aligners correct? This method works well to correct mild to moderate crowding or spacing. If your back teeth already fit together as they should, the system may be ideal. If you have an extreme overbite or underbite, braces might work better.

Are clear aligners an alternative for everyone? Clear aligners are recommended for adults and recently, teenagers. They are not usually recommended for young children.

Why is it important to have your teeth straightened? Besides the obvious benefit of feeling better about yourself and your appearance, straight and well-aligned teeth work better. You will experience a better-functioning bite and can improve your oral health.

Call us today at 518-237-0019 to make an appointment to discuss your questions about orthodontics and clear aligners.
There's a lot to like about dental implants, today's state-of-the-art tooth-replacement system. We consider them the best choice for replacing missing teeth because implants are:

Healthy
You may not realize this, but when a tooth is missing, the bone underneath it begins to melt away. That's because bone needs constant stimulation to rebuild itself and stay healthy, and it receives this stimulation from teeth. It's a delicate balance that's disrupted by the loss of even one tooth. Because implants are made of biocompatible titanium, they actually fuse to the bone and prevent bone loss.

Strong
The fusion of implant to bone is an extremely solid connection. Not only does it offer a strong replacement for a missing tooth, but it can also offer support to other tooth-replacement methods such as fixed bridges or dentures. By themselves, these other methods would not preserve bone and might even hasten its loss in the case of dentures. But with implants, bone-loss is prevented — as is embarrassing and uncomfortable slippage of dentures.

Aesthetically Pleasing
When you receive your implant, it is left alone for a few months to complete the fusion process described above. Then it is topped with a crown made of a realistic tooth-like material. The result is so convincing as a tooth replacement, only you and your dentist may be able to tell it's not a natural tooth.

Long-Lasting
Dental implants have an amazing success rate — over 97%. And once they fuse to the jawbone, they should never need replacement. In fact, they will likely outlast the crowns to which they are attached, but this is not a problem. Implant crowns are precision components that detach for easy replacement, should the need arise.

Economical
Implants have a higher initial cost than other forms of tooth replacement, but when you consider how long they last, they are very economical. Consider it an investment in your health, appearance and self-confidence.

Getting enough sleep is necessary for good health. We all know how energetic we feel when we are sleeping well at night. Yet, many of us do not feel rested, even after seven or eight hours of sleep. Let's answer some common questions about snoring and sleep apnea, problems that are often called sleep related breathing disorders (SRBD).

What is the purpose of sleep?
Scientists know we need sleep, at a particularly deep level, to be rested, but they are not sure why we need sleep. Sleep may have evolved as a way to conserve energy in the body, to conserve food supplies, or to reduce our risk during darkness. Sleep appears to give the brain a chance to store and organize its information and the body a chance to recuperate. Sleep studies have shown that in order to get the full benefits of sleep we need to sleep long and deeply enough to enter into a series of sleep cycles including Rapid Eye Movement (REM) and Non-Rapid Eye Movement (NREM) sleep.

What kinds of problems get in the way of the type of sleep we need?
There are eight main categories of sleep disorders, but the ones affecting the largest numbers of people are insomnia, SRBD, and Circadian Rhythm Sleep Disorders. SRBDs include snoring and Obstructive Sleep Apnea (OSA), which is a serious health problem.

How do I know if I have OSA or another SRBD?
Often, your bed-partner will tell you that you snore. Chronic loud snoring is an indicator of OSA. To make a diagnosis your physician must take a thorough sleep and medical history. The diagnosis may then be confirmed by a study in a sleep lab.

What causes sleep apnea or OSA?
Snoring and OSA happen when your tongue and other soft tissues in the back of your throat collapse backwards and block airflow through your upper airway or windpipe. You may briefly awaken as many as 50 times per night because of these breathing lapses. These brief awakenings, called micro-arousals, keep you from reaching the deep stage of sleep your body needs.

What are the treatments for sleep apnea?
Treatments include CPAP therapy, in which patients wear a mask while sleeping. The mask pushes air through the airway, keeping it open. In Oral Appliance Therapy (OAT) patients wear a device that moves the lower jaw forward, allowing more room for air to move down the airway. Oral surgical procedures and orthodontic approaches also have the goal of moving the tongue away from the throat.
Dental implants are replacements for missing teeth. They are very stable and can be made to look as good as or better than the teeth they replace. How do we do it? Here are seven frequently asked questions.

What are the parts of a dental implant?
The implant consists of a root, usually made of a titanium alloy, which extends below the gum tissue into the bone; and a crown, which emerges from the gum and resembles the crown of the original tooth.

Why is a dental implant so stable?
Titanium has a property of fusing with the bone of the jaw, so that it actually becomes part of the bony structure. The new implant's stability depends on having the needed volume of bone and gum tissue in the right position to anchor the implant.

How can you make sure I have enough bone?
When a tooth is lost, the bone in which it was anchored will resorb or melt away if care is not taken. It is important to minimize trauma during tooth removal to preserve bone tissue. If tissue has been lost it can be built up by bone grafting techniques.

What factors make a crown on an implant look real?
How real the crown looks depends on its shape, particularly as it emerges through the gum tissues, its color and its position relative to the teeth around it.

What is the emergence profile?
This term refers to the way the crown emerges through the gum tissue. It involves both the shape of the implant and how far it is placed into the gum and bone tissues.

How do you match the color of the crown?
We analyze your tooth color using shade guides and/or photography to provide the dental lab with as much information as possible to create the best color match. This is part of the artistry of reconstructive dentistry.

How will my gums look with my dental implant in place?
When people use the word “gums” they are often referring to the small pink triangles of tissue that fill in the spaces between teeth, called “papillae.” An implant must be placed at the correct distance from adjacent teeth and at the correct depth below the gum tissue for natural looking papillae to form.

You can see that success in matching of color, shape, and location of an implant is not simple and depends on the skill, artistry, and experience of your dental team.

Contact us today at 518-237-0019 to schedule an appointment to discuss your questions about dental implants.
All during February, we are taking part in the celebration of National Children's Dental Health Month. This annual observance is sponsored by the American Dental Association (ADA) to raise awareness of how important it is to develop good oral hygiene habits at an early age. Good dental health will promote your children's overall health and even their performance at school. According to the U.S. Surgeon General, an estimated 51 million school hours are lost per year due to dental-related illness.

To get every child off to the best possible start, we recommend that the first dental visit take place around the time of your child's first birthday. This will establish a “dental home” for your little one and allow caring professionals to respond to any issues that come up early on. Regular, semi-annual visits should follow to maintain healthy baby teeth, which in turn will have a positive affect on the permanent teeth that will replace them.

If your children are of school age, they will probably enjoy some of the games, puzzles and coloring sheets the ADA has created to help instill healthy habits. You can access the whole collection at http://www.ada.org/5578.aspx#sheets.

If you have any questions about oral health for children, please contact us today at 518-237-0019 to schedule an appointment for your child.

Florence Henderson is a multi-talented actress most recognized for her role as Carol Brady on The Brady Brunch, one of the longest-running situational comedies. In fact this role earned her the title of America's Favorite TV Mom and her first TV Land Pop Culture Icon award, which is on permanent display in the National Museum of American History.

During an interview with Dear Doctor magazine, Henderson discussed her oral health as well as her role as spokesperson for Polident (denture cleanser) — even though she does not have dentures. Henderson attributes her beautiful, natural smile to prevention. “Flossing, brushing and regular dental check ups are vital if you want to keep your teeth,” she said, adding, “I always have mouthwash, dental floss, toothpaste and a toothbrush on the set.”

Similar to the great advice “Carol Brady” shared on television, Henderson's advice on oral hygiene is spot on. We agree that an effective educational approach to oral hygiene and diet is essential to keeping teeth for a lifetime.

The first step is to ensure you have a proper brushing and flossing technique. We can go over these during your next office visit. Our goal is to ensure that you are applying the ideal amount of pressure and motion because gum tissues are soft and can easily be damaged. And you should never use a hard-bristled toothbrush or saw at your gums and teeth when brushing. The best technique is a modified, gentle scrub where you hold a well-designed, multi-tufted toothbrush at a 45-degree angle to the gum line to gently wiggle/scrub your teeth clean.

As for flossing, you should do it at least once daily to remove the plaque buildup that occurs in the protected areas between teeth where your toothbrush can't reach and where periodontal (gum) disease and dental caries (cavities) start and progress. Many people are shocked to learn that over 50% of the accumulation of plaque occurs in these areas.

To learn more about proper oral hygiene, you can continue reading the Dear Doctor magazine article “Oral Hygiene Behavior.” Or you can contact us today at 518-237-0019 to schedule an appointment so that we can conduct a thorough examination and discuss what treatment options will be best for you.
As the youngest person ever to host Entertainment Tonight, Maria Menounos, an independent filmmaker, actress, and co-host of daily entertainment news program Extra, has made a huge splash in the world of entertainment journalism. However, she is also an avid ambassador for the American Diabetes Association, a cause that is very dear to her heart because her father is a diabetic.

Her father's illness taught Menounos and her family about the importance of maintaining good general and dental health. This included a diet packed with fruits and vegetables, many of which they raised themselves. According to Menounos, they also ate little-to-no junk food. These habits still help keep the busy celebrity journalist fit and smiling with beautiful, healthy teeth.

Speaking of her smile, Menounos openly discusses her oral health in her interview with Dear Doctor magazine. She has had no major dental enhancements — not even braces — but does occasionally brighten her smile with tooth whitening. She also feels that her teeth are healthy due to the sealants she had as a child.

We could not agree more with Maria! Sealants for the tiny grooves in teeth known as “pits and fissures” are something that every parent or caregiver should consider for their children. The enamel of newly erupted teeth is more permeable, meaning that the acids produced by bacteria in the mouth can damage these teeth more easily, making them more susceptible and less resistant to decay. The good news is that dental sealants help protect teeth until the enamel has matured. Because of sealants — along with fluoride, good hygiene, and better nutrition (including less sugar consumption), tooth decay has been dramatically reduced.

If you are interested in learning more about dental sealants, contact us today at 518-237-0019 to schedule an appointment so that we can conduct a thorough examination. During this private consultation, we will also discuss any questions you have as well as what treatment options will be best for you or your children. However, to learn more about dental sealants now, you can continue reading the Dear Doctor magazine article “Sealants for Children.” And to read the entire interview with Maria Menounos, please see the Dear Doctor magazine article “Maria Menounos.”

Supermodel Christie Brinkley has a one-in-a-million smile, but she is just one of millions who have benefited from today's preferred tooth-replacement technique: the dental implant. In a wide-ranging interview, Brinkley told Dear Doctor magazine about a helicopter accident she suffered while back-country skiing.

“I fractured two molars in the back of my mouth and I had to get two dental implants,” Brinkley told the magazine. “I am grateful for the dental implant technology that feels and looks so natural.”

You might think it serves little purpose to replace a missing back tooth that was barely visible in your smile to begin with — especially if you don't spend a lot of time posing for magazine covers. But this is actually not the case. Your molars are extremely important for chewing and even for maintaining a more youthful appearance.

Dentists generally agree that losing posterior (back) teeth can have many consequences for the remaining teeth and their surrounding structures, i.e., bone and gums. If back teeth are missing, the front teeth end up bearing more stress than they were built for. And there are certain things that happen when any tooth is lost, whether front or back, that can affect function and appearance.

For one thing, when a tooth is lost the adjacent teeth tend to drift into the empty space or tip towards it. This can adversely affect your bite. Too much shifting can render a tooth basically useless and also leave it more vulnerable to gum disease.

Another complication is the loss of tooth-supporting bone that inevitably occurs when teeth are lost. When a tooth comes out, the bone under it actually begins to melt away. Since back teeth support the vertical dimension of the face, their loss can cause what's known as “bite collapse” — a reduction in facial height that becomes increasingly noticeable over time and can make you look older.

A dental implant can prevent all of these things, while providing you with a replacement that looks and feels just like the tooth you lost.

If you are interested in learning more about implants, please contact us today at 518-237-0019 to schedule an appointment for a consultation.
